More about this coverage
- You are eligible if you are a credit union member age 20 through 74. If your spouse
is also a credit union member age 20 through 74, he or she can also enrol for no-cost
coverage.
- If you would like to purchase additional coverage, your spouse doesn't need to be
a credit union member to enrol, but must be age 20 through 74. He or she can be
insured when you enrol.
- Your beneficiary will automatically be your estate for the no-cost coverage. If
you choose to enrol for additional coverage, you can name any beneficiary you like.
If you select additional coverage for your spouse, you will automatically become
your spousal beneficiary.
- Your additional coverage comes at affordable group rates. Thanks to your credit
union membership, premiums for this coverage are very reasonable at any age!
- Your benefit will be paid for death due to accident within 180 days of the accident
date, provided the accident occurs while the Certificate and Master Group Policy
is in force.
- The full amount will be paid if you are insured and under 75. The benefit will be
50% for accidental death between ages 75 and 80.

Coverage
Your coverage has some exclusions. Benefits will not be payable for death resulting
directly or indirectly from any of the following causes: self-destruction or self-inflicted
injuries, whether sane or insane; committing, attempting or provoking an assault
or criminal offense; war, insurrection, acts of terrorism, riot or hostilities of
any kind, whether declared or undeclared and whether or not the Life Insured was
actually participating therein; bodily or mental infirmity, illness, disease or
bacterial/viral infection; poisoning in any form or inhalation of gas or fumes,
whether voluntary or involuntary; the taking of any drug, except as prescribed by
a physician; the driving or operation of any vehicle while intoxicated; or travel
or flight in any aircraft (a) as a student pilot or member of the crew or (b) for
testing or experimental purposes or (c) under any military authority or (d) for
the purpose of parachute descent. The Accidental Death Insurance Plan described
in this summary is underwritten by Co-operators Life Insurance Company ("Co-operators Life").
In this summary, we have attempted to explain clearly and briefly the benefits available
to you as a Credit Union member. All the provisions of the Plan are contained in
the Master Group Policy No. 850-5213-8 issued by Co-operators Life. Since the Master Group
Policy is complete in detail, the final interpretation of any specific provision
or claim is governed by it. You may contact your Credit Union to review a copy of
the Master Group Policy.
